SPORTS INJURY AND PERSONAL ACCIDENT
1. The injured member reports the injury to the appropriate official as soon as possible after sustaining the injury and obtains a sporting injury claim form, from their local Association or the web site listed above.
2. The injured member completes the claim form, the form is then signed by the local Association.
3. The claim form signed in section 2 above, along with all supporting documentation is then forwarded to the state head office for verification of Registration Status and signed prior to forwarding to the Insurer.
4. The state head office then sends the claim form (with all documents attached) to American International Group
